Re-Test

If you did not pass the CWCA®, CWS®, or CWSP® exam and would like to re-test, please submit this form to the AAWM office a minimum of 15 business days prior to your desired testing date.

Candidates who wish to retake an exam must wait 90 days after their previous exam date before their new three-month eligiblity window begins. Candidates may submit a Re-Test Application Form prior to the end of the 90 day waiting period; the new three-month eligibility window will start 90 days after their previous exam date.

10-Year Recertification

All Associates and Diplomates of the AAWM are required to recertify their credential every 10 years on or before anniversary of their initial certification date. Please use this form if it has been 10 years since you last sat for the National Board Certification Examination, and you wish to maintain your certification.

Fill out the form from your computer. Then print, sign, and submit via email, fax, or mail. You will be given a 3 month window to schedule and sit for your exam upon processing.

Your three-month eligibility window will begin immediately upon application approval. Do not submit your application until you are prepared for your three-month eligibility window to begin. Window extensions will not be granted.

Reschedule

If you are unable to sit for the examination for any reason, AMP will allow you to reschedule your examination one time, at no charge, if you provide advance notice a minimum of two business days prior to your previously scheduled exam date. If you do not provide advance notice a minimum of two days prior to your previously scheduled exam date, and do not sit for your examination, you will have forfeited your application and all associated fees. If, for any reason, you need to reschedule your examination for a second time, or any additional times, please submit this form, with your $150 Reschedule fee, to the AAWM. Your Reschedule Form will be processed and you will be able to reschedule your exam after approximately 15 business days.

Note: If you have missed your exam appointment you may NOT use this form to reschedule your exam. If you have missed your exam appointment, you have forfeited your application and all associated fees. You will need to submit a new application in its entirety.

AAWM Appeals Policy

This form outlines the appeals process for adverse decisions affecting certification as a CWCA®, CWS®, or CWSP®.  The following adverse decisions affecting eligibility or certification by the American Wound Management Organizations are subject to appeal:  adverse administrative decisions, adverse substantive decisions, adverse disciplinary decisions.

NO APPEAL may be taken from an adverse decision based on an individual’s receipt of a failing grade on an AAWM examination absent extraordinary circumstances, as determined solely by the AAWM

AAWM Disciplinary Policy

This form contains information regarding disciplinary actions based on irregular behavior, fraudulent credentials, legal, regulatory or credentialing action, or documented allegations of misconduct.

International Testing Addendum Form

Please use this form if you wish to sit for the CWS® or CWSP® exam at an Assessment Center outside of the United States. Submit this form with your Application or Re-Test Application form. You will indicate your preferred testing location when scheduling your exam with our testing company AMP, a PSI Business. Scheduling is subject to availability.

Retirement Status

Use this form to apply for Retirement Status with the AAWM. Applicants must meet all qualifications for Retirement Status.
